About Colorado Access Choice

Colorado Access Choice is a Colorado-based health insurance carrier offering individual health plans on and off the Colorado health insurance marketplace. Coverage, benefits, and costs are described in your plan's Evidence of Coverage (EOC), Summary of Benefits and Coverage (SBC), and other plan documents.

Network and provider directory

Provider network information is updated regularly, but participation can change. Before receiving care, confirm that a provider is in-network by checking the current provider directory or contacting member services.

Prescription drug coverage

Covered drugs are listed in the plan formulary. The formulary may change during the plan year in accordance with applicable law. Notice will be provided as required.

Prior authorization and utilization management

Certain services require prior authorization to be covered. Utilization management decisions are based on the appropriateness of care and existing coverage, not on financial incentives for reviewers.

Grievances and appeals

Members have the right to file a grievance or appeal a coverage decision. Instructions and forms are provided in your plan materials and on the Members page.
